Principle Accountabilities Include:
• Plan, monitor, evaluate, and report all supported community living needs for assigned clients, including facilitating staffing, reviewing and monitoring plans, and completing timely documentation.
• Organize and assist in implementation of client staffings
• Attend and provide input into client staffings, advocating for client’s goals, needs, and program services
• Develop and maintain individualized program plans, including monitoring progress, providing feedback, and changing plans when necessary
• Provide scheduling support to team members and clients daily as needed and for monthly schedules.
• Maintain a working knowledge of assigned clients’ profiles, program plans, and care team members.
• Active communication with funding sources, family, clients, and other team members.
• Support clients with activities of daily living, including but not limited to: safety, social and leisure skills, housekeeping, housing, medical appointments, shopping, cooking, eating, transportation, and personal hygiene counseling.
• Support clients through crises which may occur outside of business hours.
• Provide adaptations to individualized activities and group programming as necessary to teach goal skills.
• Assist with planning program options for group SCL activities.
• Maintain active communication with internal team members and assigned clients’ support teams.
• Provide referrals and establish linkages with community agencies and services as needed by client.
• Attend all team meetings, providing feedback and solutions, as applicable, to quality program development. Take notes at team meetings as needed.
• Assist with program evaluation throughout the year as requested.
• Perform other duties as assigned.
Requirements:Knowledge, Skills, and Experience Requirements:
• Bachelor’s degree in human services or related field and/or two years of experience working with people with disabilities preferred.
• Experience working with persons with disabilities preferred.
• Ability to work independently and make quality decisions.
• Ability to bend, stoop, twist, and lift.
• Ability to transfer or move a minimum of eighty pounds.
• Access to reliable transportation for regular participation in work
• Ability to maintain a good driving record.
• Insurable under Easterseals vehicle insurance policy
• Ability to work non-traditional hours as required, including weekends and evenings.
• Ability to work cooperatively with a variety of people, with and without disabilities in a professional manner.
• Ability to work in a team environment.
• Ability to communicate effectively in written and verbal formats.
• Ability to operate agency databases.
• Experience in word processing software; Word and Excel preferred.
• Access to a telephone for on-call system-
• Ability to successfully pass Adult and Child CPR, First Aid, and Brain Injury Waiver training within the first 90 days.
• Ability to successfully pass Mandatory Reporter of Child and Dependent Adult Abuse within the first 30 days.
