Work hours: Full Time Days (8:00am-5:00pm) for 40 hrs./week (Hybrid work allowed at manager's discretion)

What You Will Do:

Collaborates with organization-wide cross-functional teams in planning, designing, implementing, and maintaining improvement plans.
. Support THPG in the assessment, development, and planning to ensure data supports performance objectives.
. Run/develop reports utilizing discrete data and conducts analysis for purpose of measuring and identifying trends/patterns that represent opportunities for improvement and increase operational efficiency.
. Understand the various and complex data sources, interfaces, that perform transactions between applications (PRIME Dashboard, THPQ Quality Dashboard, and CC1).
. Maintains data for monthly/quarterly/yearly statistics and reports for leadership.
. Develops auditing tools and definitions for consistent collection of data for accurate reporting.
. Obtain and use benchmark data from internal and external sources to identify opportunities for improvement of clinical outcomes, patient safety outcomes, and track progress towards performance goals.
. Provide direction and oversight in the creation, dissemination, and analysis of regular reports of outcome data and measures including progress towards goals.

. Ongoing development of initiatives to provide education and awareness to leadership (DIRs, PMs, etc.) regarding incentive measures, dashboard utilization/functionality, and changes or updates to THPG quality measures and performance.
. Collaborate and conduct monthly and PRN sessions with APPs and physicians to provide coaching on both THPG Quality Dashboard and SWHR Provider Performance Dashboard.
. Lead Epic HCC Dashboard education for new Primary Care APPs.
. Provide dashboard education with Practice Managers and Directors with in-depth walkthroughs of best practices, workflows, quality measures, and access troubleshooting.

. Tracks and trends provider performance and provides reports to leadership.
. Collaborates with THPG ITS to review dashboards, ad-hoc reports, and statistical reporting to support enterprise-wide performance.
. Takes appropriate action to improve operations based on patterns and trends.
. Provides prompt customer service with internal and external customers.

. Develops presentations and present trends, including physician-focused meetings.
. Makes recommendations on process improvements based on research of industry best practices.
. Manages process improvement initiatives, documenting requirements and working with associated business units to deliver.
. Assists building relationships and responses to internal (THPG Primary Care Practice Managers, Directors, Physicians, Executive Leaders) and external customers (SWHR Clinical Operations and Data Analytics).
. Maintain relationships with outside vendors (Exact Sciences, contracted health plans, etc.) to stay abreast of current and future initiatives that benefit quality improvement and potential cost savings for the organization.
. Makes recommendations to leadership and assists in implementation and providing feedback.

. Ensure proper access is granted, assist with troubleshooting issues and escalate to SWHR Data Analytics when needed.
. Maintain open access issue to ensure timely follow up and resolution is conducted.

What You Need:

Education
Bachelor's Degree Health Information Management, Health Informatics, Business, or Healthcare related field. Req Or
Associate's Degree Business or Healthcare related and 2 Years Relevant Medical Practice Management or ACO Population Health Management or CMS Quality P4P or MIPS experience Req Or
H.S. Diploma or Equivalent and 4 Years Relevant Medical Practice Management or ACO Population Health Management or CMS Qualiy P4P or MIPS experience Req

Experience
1 Year Experience in population health management and reporting Pref and
1 Year Experience in a health care environment with EMR Req

Skills
Strong analytic, problem-solving skills, and attention to detail.
Knowledgeable in pulling, compiling, and reporting statistical data.
Able to employ effective problem-solving skills to make appropriate recommendations for process improvement and/or training opportunities.
Ability to write detailed reports and/or presentations of findings and recommendations.
Excellent interpersonal, verbal, and written communication skills both up, and down the chain of leadership.
Ability to develop and maintain positive relationships with providers and other stakeholders.
