General description/ Qualifications:
The Case Management Referral Specialist is a position that assists children, adolescents and adults and their families as the access point to Administrative and Blended Case Management Services at Penndel Mental Health Center. The Referral Specialist operates as Penndel Mental Health Center’s Case Management liaison to hospitals, community members and other network providers who wish to inquire as well as refer individuals to Mental Health Case Management. The Referral Specialist receives, tracks, processes and intakes referrals to case management based on client’s needs and eligibility.
Requires a minimum of a Bachelor’s degree in human service field. Blended Case Management experience strongly preferred.
Duties/Responsibilities:
- Receives, processes, and tracks incoming ACM and BCM Case Management referrals.
- Coordinates with referral sources to schedule interviews and assess referred clients
- Assess referrals for eligibility for Blended Case Management services
- Determines the appropriate case management department for new intakes (ACM or BCM)
- Assists potential referrals by providing coordination to community resources if case management services are mutually determined to not be necessary
- Conducts the initial intake and assessment of case management clients including the completion of the initial matrix to identify case management level of care (Resource Coordination or Intensive Case Management)
- Collaborates with the Associate Executive Director regarding potential appropriateness of referrals for higher levels of community care (such as Assertive Community Treatment) or more appropriate case management services depending on cognitive functioning (such as Intellectual Disability Supports Coordination)
- Fulfills other record keeping and reporting requirements
- Attends agency staff development and training meetings on a weekly basis, monthly BCM trainings, and OMH training sessions and seminars specifically designed for BCM
- Participates in regularly scheduled supervisions which include: individual once per week, and group supervision
- Performs related duties as required/assigned
- Must adhere to Corporate Compliance policy
- Position normally requires the physical demands of performing work requiring low physical exertion, lifting up to 10 pounds and mobility that would enable employee to attend meetings in a variety of settings.
Physical Requirements:
Equipment use including but not limited to computer (including repetitive motions of typing, seeing monitor), copier, fax, printer, shredder, telephone, cell phone, and voice mail and light lifting up to 10 pounds, ability to drive agency vehicles.
