Job Description

Job Summary:

  • Performs under the direct supervision of the Medical Director of Palliative Care and is accountable for planning, implementing, evaluating, and integrating Palliative Care (PC) activities.
  • Coordinates the day-to-day activities of the department by maintaining a database for all patients on PC service. Responsible for providing coordination of the referral process for PC.
  • Coordinates advance care planning, advance directives and/or living wills for the PC patients and family members.
  • Participates in the initial screening and intake of referrals for PC services.
  • Analyzes data to determine program direction by maintaining a data base of consults, visits, and discharges.
  • Works closely with post-acute care services in the Forrest Health System. Works closely with dc planners to enhance coordination of palliative care services post discharge.
  • Improves patient care processes and outcomes by utilizing evidence-based approaches with the input of the PC Medical Director. Provides education to physicians and staff accordingly.
  • Guides the PC interdisciplinary committee.
  • Collaborates with administrative and clinical leaders to resolve issues.
  • Conducts internal marketing and education that promotes utilization of PC throughout Forrest Health System.
  • Coordinates the development of policies and protocols for grief, bereavement and emotional needs in collaboration with Patient Care Services and the PC Medical Director.
  • Demonstrates knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs.
  • Performs other job duties as needed.

Performance Expectations:

  • Demonstrates the ability to triage patient consults for palliative care and to organize daily workload/activities for the department.
  • Provides counseling, guidance and assistance with development of advance care planning, advance directives, and/or living wills.
  • Demonstrates knowledge and competency in treatment of palliative care patients.
  • Demonstrates ability to collaborate with interdisciplinary team members in providing patient care.
  • Demonstrates understanding of program development.
  • Demonstrates ability to improve the quality of patient care, and reduce gaps in care.
  • Demonstrates the ability to provide education, training and development of hospital staff in concepts of palliative care.
  • Demonstrates the ability to handle multiple tasks and prioritize work.

Qualifications:

Education/Skills

Graduation from an approved non-online RN program required. Bachelor degree required. Management and/or Leadership experience preferred.

Work Experience:

A minimum of five years of diverse clinical nursing practice required.

Certification/Licensure:

Required to hold a current license to practice professional nursing in the State of Mississippi.

Additional Certification/Licensure - Obtained based on required timeframe below

  • Basic Life Support
