As a** Physical Therapist**, you will be a key member of our care team, responsible for assessing, planning, and delivering therapy treatments that promote independence, safety, and functional ability—primarily for geriatric or frail populations.

Key Responsibilities: Conduct therapy assessments Develop personalized care plans in collaboration with the interdisciplinary team (IDT) Deliver and document therapeutic interventions Perform treatments such as active/passive exercises, muscle re-education, gait training, and activities of daily living (ADLs) Apply therapeutic modalities (heat, electrical stimulation, massage, etc.) Supervise Therapy Assistants and support Functional Maintenance Programs Educate patients, caregivers, and staff on the proper use of therapeutic equipment
