This role is centered on delivering compassionate, whole-person support to individuals navigating substance use recovery within a fully remote care environment. You will play a key role in helping clients stay engaged in their treatment journey by closely monitoring progress, identifying barriers, and coordinating timely interventions with clinical teams. The work blends care coordination, client advocacy, and continuous engagement to ensure individuals receive the right support at the right time. You will collaborate with multidisciplinary professionals in a mission-driven setting focused on dignity, recovery, and second chances. - Monitor client progress across treatment plans by reviewing engagement data such as attendance, program participation, and adherence indicators.
- Identify deviations from care plans and coordinate with clinical teams to implement appropriate interventions and support strategies.
- Assist clients in overcoming engagement barriers by providing proactive outreach, encouragement, and problem-solving support.
- Coordinate scheduling of client appointments with providers and manage rescheduling when necessary.
- Verify insurance coverage and eligibility, and support referrals to external healthcare and community resources such as housing, transportation, and employment services.
- Respond to client inquiries, provide ongoing guidance, and assist with navigating digital care platforms and technical issues.
- High school diploma or equivalent required; associate degree preferred.
- 1–3 years of experience in behavioral health, case management, or a related healthcare support role.
- Strong communication and interpersonal skills with the ability to engage diverse populations in a respectful, culturally sensitive manner.
- Excellent organizational skills, time management, and ability to manage multiple client needs independently.
- Understanding of the U.S. healthcare system, including insurance processes and care coordination workflows.
- Familiarity with electronic health records (EHR/EMR) systems is strongly preferred.
- Ability to provide emotional support, motivation, and structured client follow-up in a professional setting.
- Fluency in English required; Spanish proficiency strongly preferred.
- High level of confidentiality and discretion when handling sensitive client information.
