Manager

Job Summary

The Care Management Manager works to lead and support Care Managers at Shannon. The Care Management Manager supports the development of patient-centered, team-based care. Responsibilities include oversight of day-to-day duties of Care Managers. S/he will supervise the support of primary care physicians (PCPs) and practices in managing their panel of patients. The Care Management Manager ensures Care Managers are gathering and organizing patient data, identifying & addressing patient’s unmet needs, engaging patients in their own care, gathering summary information for treatment interventions, and enhancing ongoing communication between the patient and her/his care team.

The goal of the Care Manager is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment. The Care Management Manager also leads projects associated with population health, shared savings plans, community-based health initiatives as needed, as well as overseeing high complexity crisis intervention.

Qualifications

Education

  • Required
    • High School Diploma, GED, or equivalent

Experience:

  • Preferred
    • Two years of customer service experience
    • Two years of healthcare experience
    • Two years of leadership experience

Certification/Licensure:

  • Preferred
    • Certified Medical Assistant/Registered Medical Assistant
    • Basic Life Support (BLS) Certification
