Position Summary
The Licensed Practical Nurse (LPN) supports patient care through medication administration,
monitoring, documentation, and collaboration with RNs, medical providers, and the
interdisciplinary treatment team. This role emphasizes consistency, patient engagement, and
clinical support rather than hospital-level acuity.
Key Responsibilities
● Measure and document patient vital signs and general health observations
● Assist in monitoring patient conditions and report concerns to supervising RN or medical
provider
● Administer medications as ordered and within LPN scope of practice (oral, IM, SQ, SL)
● Supervise medication administration as assigned and in accordance with facility policy
● Observe for and report adverse medication reactions; document appropriately in EMR
● Maintain accurate, timely documentation in the electronic medical record
● Assist with basic wound care and patient health education as directed
● Support chart completion and documentation accuracy
● Collaborate with RNs, providers, and program staff to support patient safety and care
continuity
● Follow Freeman Recovery Center policies, procedures, and OSHA safety standards
● Complete required training and ongoing education

Qualifications
● Active Licensed Practical Nurse (LPN) license in good standing with the State of
Tennessee
● One (1) year of nursing experience preferred (new graduates encouraged to apply)
● CPR and First Aid/AED certification required
● Ability to accurately document in an electronic medical record (EMR)
● Strong communication, organization, and interpersonal skills
● Ability to maintain confidentiality and professionalism at all times
