Job Description

Job Summary:

The Transfer Center/Patient Placement Coordinator performs under the direct supervision of the Admin Director of Nursing Operations. The Coordinator will be responsible for ensuring the daily operation, leadership, and performance within the Admission/Discharge Department run smoothly and efficiently. The role oversees patient placement, interfacility transfers, and bed assignment processes to ensure timely, safe, and efficient patient flow across the health system. The Coordinator will collaborate closely with nursing leadership, physicians, and external referring facilities to optimize access, capacity, and patient experience. The Coordinator demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also shows knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care required. Performs other job duties as assigned.

Performance Expectations:

  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs, and to provide the care needed.
  • Demonstrates thorough, concise, and timely documentation, shift reports and verbal communication. Communicates pertinent patient information to all health care team members.
  • Demonstrates critical thinking skills and uses professional judgement to respond promptly in assisting proper transfer of patients.
  • Communicates effectively with others in an appropriate and timely manner. Adheres to and promotes the established values of the organization, i.e. customer service, safety, infection control and compliance standards.
  • Consistently follows patient care and FGH policy, procedures, and standards of care in performing nursing duties.
  • Communicates with facilities to provide a smooth transfer process for patients being received to FGH or being sent to another facility. Collects the appropriate data for transfer; including demographics and report of patient’s current physical condition, vital signs and accepting physician.
  • Provides for exceptional patient and coworker experiences by following our standards of practice of always using positive language .
  • Demonstrates independence and exercises sound judgment in interactions with physicians and the interprofessional care team.
  • Exhibits open and professional communication.
  • Role models fair and consistent leadership skills for effective team dynamics.
  • Performs all the duties of required of a Admission/Discharge staff nurse.
  • Communicates timely and competently with all employees and providers.
  • Confirms that employees follow hospital policies, protocols, procedures, safety, and infection control practices.
  • Assess the performance of staff, counsels’ employees concerning performance, and provide feedback to the Admin Director of Nursing Operations
  • Performs employee’s yearly evaluations in Health Stream
  • Utilizes KRONOS, Lawson, Health Stream and EPIC efficiently
  • Works with Admin Director of Nursing Operations to update policy, procedure, and protocols
  • Promotes a positive work environment
  • Participate in on-call rotation as needed for operational coverage or escalation management

Qualifications:

Education/Skills:

Graduate of a recognized non-online RN program required. BSN preferred.

Work Experience:

5 year medical/surg nursing care preferred, but not required. 1 year of Transfer Center experience required.

Certification/Licensure-DUE UPON HIRE

  • Licensed RN able to practice within the State of MS

Additional Certification/Licensure - Obtained based on required timeframe below

  • Basic Life Support

Within 30 Days of Employment

Required

  • Advanced Cardiac Life Support

Within 90 Days of Employment

Preferred

  • NIH Stroke Scale

Within 90 Days of Employment

Required

demonstrate initiative, set priorities, and function under stress.
