Licensed Practical Nurse (LPN) – Wound VAC Care (Wednesdays & Fridays) 19401 at Affinity HealthCare Solutions Skilled Care LLC – Norristown, Pennsylvania

We are hiring a skilled Licensed Practical Nurse (LPN) to provide Wound VAC therapy and wound care support for a home health patient requiring services twice weekly- Wednesdays and Fridays. This is a consistent, ongoing assignment ideal for an LPN with strong wound care skills, excellent assessment abilities, and confidence working independently.

Key Responsibilities
- Perform Wound VAC dressing changes and equipment management in accordance with physician orders and agency protocols.
- Conduct wound assessments, identify signs of infection or complications, and report changes to the RN Case Manager or physician.
- Provide education to the patient and/or caregivers regarding wound care, equipment use, and infection prevention.
- Accurately document all care in Axxess (or EMR provided) on the same day of service.
- Communicate professionally with the care team to ensure continuity of care and timely follow-up.
- Adhere to CHAP standards, state regulations, and agency policies.

Qualifications
- Active Pennsylvania LPN license in good standing.
- Wound VAC experience required
- Previous home health or facility-based wound care experience strongly preferred.
- Strong attention to detail, good communication skills, and reliability.
- Must be available every Wednesday and Friday.
