Job description:

Fellowship Therapy at Home is growing in Somerset County, NJ, and we're seeking a compassionate motivated Physical Therapist who wants flexibility, autonomy and meaningful one on one patient care.

Are you a compassionate and driven Physical Therapist ready to make a difference? At Fellowship Therapy at Home, we’re redefining outpatient therapy by bridging the gap between home health and clinic-based services. We provide care in clients’ homes—delivering therapy where it matters most.


This is a full-time position ranging from 30–40 hours per week based on caseload.

What You’ll Do

  • Deliver personalized, high-quality outpatient therapy in home and community settings
  • Create positive, engaging experiences that empower clients to achieve their goals
  • Ensure compliance with documentation and care standards
  • Collaborate with a multidisciplinary team to optimize outcomes
  • Educate clients and caregivers on safety, exercise programs, and wellness strategies

Qualifications

  • Current New Jersey Physical Therapist License (Required)
  • Minimum 1 year of outpatient or home health experience (Preferred)
  • Strong communication and time management skills
  • Ability to travel between client locations throughout the day
  • Comfortable working independently
