POSITION INFORMATION

Position summary:
-
The PGY-2 Pharmacy Graduate Resident in Transitions of Care will expand their knowledge in clinical pharmacy functions. They will optimize direct and indicrect patient care in a variety of settings by designing, recommending, monitoring and evaluating patient specific pharmacotherapy.

During this one year program, the resident will have frequent contact with physicians, nurses, pharmacists, community care providers as well as patients and their familiies.

MINIMUM QUALIFICATIONS

Education:
-
Doctor of Pharmacy degree from an ACPE-accredited pharmacy school
-
Completion of an ASHP-accredited Pharmacy Practice Residengy (PGY - 1)

Required length and type of experience:-
1 year PGY-1 residency required

Required licensure, certification or registry:-
Licensed Pharmacist in the State of Ohio
