POSITION INFORMATION
  • Position summary:
    • The PGY-2 Pharmacy Graduate Resident in Transitions of Care will expand their knowledge in clinical pharmacy functions. They will optimize direct and indicrect patient care in a variety of settings by designing, recommending, monitoring and evaluating patient specific pharmacotherapy.
    • During this one year program, the resident will have frequent contact with physicians, nurses, pharmacists, community care providers as well as patients and their familiies.
MINIMUM QUALIFICATIONS
  • Education:
    • Doctor of Pharmacy degree from an ACPE-accredited pharmacy school
    • Completion of an ASHP-accredited Pharmacy Practice Residengy (PGY - 1)

  • Required length and type of experience:
    • 1 year PGY-1 residency required

  • Required licensure, certification or registry:
    • Licensed Pharmacist in the State of Ohio
