LMSW Pay Range:$30.60-$50.49*

LCSW Pay Range: $39.93-$65.88*

CHMC –Wellness Center-LMSW – Full Time- Day Shift –8a-4:30p- Requisition 7543

The Social Worker functions as a core member of a collaborative care team and as the mental health care coordinator. He/She is responsible for supporting mental health care provided by primary care providers in consultation with a psychiatrist and other mental health providers as appropriate. He/She is also responsible for coordinating referral to clinically indicated services outside the primary care clinic (e.g. social services, mental health specialty care, and substance abuse treatment).

Qualifications: Master's degree in Social Work required (LMSW or greater). Must be licensed by the state of

New York. Licensed Clinical Social Worker preferred. Completed coursework or training in identification and reporting of child abuse and must be a NYS mandated report. Must maintain current BLS certification. Must be at least twenty-one (21) years of age. Must possess good moral character.
