About This Position

  • JOB SUMMARY
    • Post-graduate year two (PGY2) Infectious Diseases Pharmacy Resident will complete required and elective experiences in infectious diseases and antimicrobial stewardship. PGY2 residency programs build upon Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency training to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives for advanced practice areas. Residents who successfully complete PGY2 residency programs are prepared for advanced patient care or other specialized positions, and board certification in the advanced practice area, if available.
  • DUTIES AND ESSENTIAL FUNCTIONS
    • Overall performance rating of below indicated essential job duties:
    • Designs, executes, and reports results of investigations of pharmacy practice-related issues.
    • Establishes a collaborative working relationship with physicians and other health care providers in the health system.
    • Designs, recommends, monitors, and evaluates patient-specific therapeutic regimens that incorporate the principles of evidence-based medicine.
    • Demonstrate ownership for the welfare of the patient by performing mediation related patient education, medication reconciliation, and assistance with optimizing the patient’s drug regimen prior to discharge or in a clinic visit
    • Provides concise, applicable, comprehensive, and timely responses to requests for drug information from patients, health care providers, and the public.
    • Documents direct patient-care activities appropriately.
    • Participates in the components of disease management: identification of need for, development, implementation, and assessment of treatment guidelines and protocols related to individual and population-based patient care.
    • Prepares and dispenses medications following existing standards of practice and the health system’s policies and procedures.
    • Participates in formulary management via Pharmacy & Therapeutics (P&T) Committee (including medication-use evaluation (MUE), class review, and new drug evaluation) as well as participate in the specialty subcommittees of P&T
    • Prepares and disseminates written drug information.
    • Participates in various planning and development programs within the department.
    • Utilizes pharmacy technical and clerical personnel effectively.
    • Provides instruction to pharmacy technicians, students, PGY1 residents, and pharmacists.
    • Follows policies, procedures, and safety standards.
    • Completes required education assignments.
    • Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
    • Performs other duties as assigned.
  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
    • Minimum requirement:
      • Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree or a PharmD degree.
      • Successfully completed a PGY1 pharmacy residency program
      • Licensed pharmacist in Kentucky and Ohio by October 31st of the residency year
    • Preferred qualifications:
      • Membership in professional organization
  • WORKING ENVIRONMENT
    • Constantly works near moving mechanical parts
    • Constantly exposed to blood and/or body fluids and infection.
    • Occasionally exposed to toxic or caustic chemicals
    • Occasionally exposed to risk of radiation
    • The noise level in the work environment is usually moderate.
  • PHYSICAL DEMANDS
    • Occasionally required to maintain in a stationary position (standing or sitting).
    • Frequently required to walk and move about.
    • Frequently required to operate a computer and other equipment.
    • Frequently required to use fine motor skills to handle and operate instruments/devices.
    • Constantly required to lift and/or move items up to 10 pounds.
    • Frequently required to lift and/or move items up to 50 pounds.
    • Occasionally required to lift and/or move items up to 100 pounds.
    • Constantly required to communicate with patients and other care team requiring talking and hearing.
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
    • Occasionally required to position self in order to meet patient needs.
