We are currently seeking a Nurse Practitioner for our Palliative Care team.

This role works in collaboration with our hospice staff and under the direction of the Director of Palliative Care to provide high-quality, patient-centered palliative care. The NP will conduct visits with new and established patients in a variety of settings—including private homes, facilities, and hospitals—while helping patients and families navigate their care with compassion and expertise.

Key Responsibilities

  • Conduct comprehensive patient visits and develop individualized plans of care.
  • Provide medical expertise in symptom management, prognosis, and palliative treatment.
  • Perform Medicare-required “face-to-face” encounters as needed.
  • Collaborate with interdisciplinary team members, attending physicians, and community providers.
  • Educate patients and families on disease process, care choices, and symptom management.
  • Serve as a resource and advocate for palliative care within the community.
  • Participate in team meetings, quality initiatives, and on-call rotation as needed.

Qualifications

  • Nurse Practitioner licensure in the Commonwealth of Virginia (required).
  • At least 2 years of experience as an NP in acute care, home health, hospice, palliative, or geriatric nursing (required).
  • Board certification in hospice and palliative medicine (preferred).
  • Fellowship in hospice and/or palliative medicine (preferred).
  • Valid driver’s license and reliable insured automobile (required).
  • Strong clinical assessment skills, independence in decision-making, and ability to work collaboratively.

Schedule: Monday through Friday, 8:30 am to 5 pm, but please note that hours outside of these may require a call on some evenings and weekends.
