Looking for a role with variety, flexibility, and real impact?

CareChoice is growing our Mobile Rapid Response Team and we’re looking for experienced Disability Support Workers who thrive in fast-paced environments and enjoy stepping in where they’re needed most.

This is a unique opportunity to support clients across Metro Melbourne and Regional Victoria during important transition periods — including hospital discharges, emergency placements, and provider changes.

You’ll work closely with our Intake and Operations teams to establish safe, high-quality supports from day one.


What You’ll Be Doing

  • Deliver person-centred care
  • Step into urgent or complex support situations
  • Assist with new client transitions and stabilising care arrangements
  • Support clients with personal care, community access, and daily living
  • Provide high intensity and complex care support (where trained)
  • Maintain clear communication and accurate documentation


What We’re Looking For

  • Certificate IV in Disability, Aged Care, Community Services, or similar
  • Experience supporting clients with complex needs and behaviours
  • Current First Aid, CPR, NDIS Worker Screening Check, and Working with Children Check
  • Victorian driver’s licence and fully insured vehicle
  • Strong communication and problem-solving skills
  • Flexibility to travel across Victoria, including occasional overnight stays
  • Nursing backgrounds highly regarded
