Senior Engineer, FHIR

This position is posted by Jobgether on behalf of a partner company. We are currently looking for a Senior Engineer, FHIR in the United States.

This role sits at the intersection of modern healthcare interoperability and large-scale public health systems, focusing on building and evolving FHIR-based solutions that support national healthcare quality reporting programs. The Senior Engineer will contribute to mission-critical platforms used by healthcare providers across the United States to submit and manage quality data. You will design, build, and optimize backend services and APIs aligned with FHIR standards, ensuring scalability, compliance, and data accuracy. The position involves close collaboration with cross-functional engineering, product, and client teams in a highly regulated environment. You will play a key role in advancing healthcare data interoperability while improving system reliability and reducing provider burden. This is a high-impact engineering role where technical depth directly contributes to better patient outcomes and more efficient healthcare systems.

Accountabilities:
  • Design, develop, and maintain backend services and APIs aligned with HL7 FHIR standards, ensuring interoperability and regulatory compliance.
  • Build and integrate RESTful APIs and healthcare data services supporting large-scale quality reporting and analytics systems.
  • Collaborate with cross-functional teams and stakeholders to ensure quality, compliance, and technical requirements are met throughout the SDLC.
  • Develop test strategies, test cases, and automation frameworks to improve coverage, reliability, and CI/CD efficiency.
  • Implement and support performance, security, and data validation testing for healthcare applications.
  • Work within Agile environments to deliver features, reduce technical debt, and ensure consistent production releases.
  • Maintain and enhance FHIR resources such as Patient, Encounter, Observation, Measure, and MeasureReport.
  • Contribute to continuous improvement by evaluating new tools, frameworks, and best practices in healthcare engineering.
  • Ensure adherence to regulatory and compliance requirements, supporting audit readiness and documentation standards.
Requirements:
  • 8+ years of software engineering experience with strong backend development expertise.
  • Hands-on experience with Java and Spring Boot, along with exposure to additional languages such as Go, Python, TypeScript, or Angular.
  • Strong experience working with HL7 FHIR standards, including FHIR R4, resources, profiles, and implementation guides.
  • Proven ability to design and build RESTful APIs aligned with healthcare interoperability standards.
  • Experience with healthcare data formats such as QRDA, C-CDA, or claims data.
  • Familiarity with Clinical Quality Language (CQL) and digital quality measurement concepts (eCQM, dQM).
  • Strong understanding of relational databases such as PostgreSQL and distributed cloud environments (preferably AWS).
  • Experience with CI/CD pipelines, Infrastructure as Code (Terraform preferred), and Agile development practices.
  • Experience working with FHIR servers such as HAPI FHIR or equivalent platforms.
  • Strong problem-solving skills with the ability to operate in regulated, compliance-driven environments.
