About the Role:

The Discharge Support Specialist supports the Care Management team by coordinating post-acute referrals, facilitating discharge planning activities, and ensuring patients receive required notifications and documentation. This role helps promote safe, timely transitions of care by collaborating with patients, families, care managers, and community providers.

Key Responsibilities:

• Support discharge logistics by arranging follow-up appointments, verifying patient information, coordinating post-discharge services, and providing administrative support to facilitate efficient patient discharges
• Ensure compliance with patient notification requirements by delivering discharge-related notices, obtaining required signatures, and maintaining accurate documentation within the medical record.
• Coordinate post-acute care referrals, track authorizations, communicate referral status, and escalate delays or barriers to care transitions.

Requirements:

• Education: High School/GED or Associate’s Degree
• Experience: 1-3 years; medical terminology preferred
