Responsibilities
Job Summary

Under the supervision of the Enrollment Manager, develops community relationships with stakeholders with the goal of generating referrals to support enrollment goals.

Essential Job Functions

  • Develop and maintain relationships with healthcare providers, community organizations, and referral sources
  • Identify growth opportunities, target audiences, and high-value partnerships within the assigned territory.
  • Conduct regular outreach visits, presentations, and participation in community events to increase awareness of PACE services and obtain referrals.
  • Maintain a diversified referral network and consistent community presence.
  • Utilize time and resources strategically to maximize outreach effectiveness and enrollment opportunities.
  • Maintain accurate and up-to-date documentation of outreach activities, referral partners, and leads in CRM.
  • Track partner engagement, follow-up activities, referrals, and outcomes.
  • Use CRM data and reporting to evaluate performance and adjust outreach strategies.
  • Ensure assigned accounts and referral partner records are complete, accurate, and regularly updated.
  • Respond promptly to referrals and leads.
  • Support the enrollment process by educating prospective participants and facilitating a smooth transition through enrollment process.
  • Track individual pipelines to create new strategic goals.
  • Through marketing efforts, generate referrals and a health pipeline to support center and enrollment needs.
  • Build trust with seniors, caregivers, and referral partners through professional and compassionate and trustworthy communication.
  • Deliver accurate and complete PACE presentations.
  • Work closely with Enrollment Specialists, Intake Specialists, center staff and leadership to achieve enrollment goals.
Qualifications

Minimum Qualifications
  • A bachelor's degree (preferred) in business, marketing, or a health or human services field, or a high school diploma and community outreach training (preferably health care specific).
  • Minimum Two years of experience in a marketing, sales and/or admissions role for a health care provider or health plan, or in an information and referral resource role. Shall have either training or related experience in the job assigned.
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record
Required Skills/Knowledge/Abilities
  • Strong verbal and written skills necessary for communication with participants, clinicians and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds
  • Ability and willingness to meet the organization’s attendance and dress code policies
  • Ability to handle confidential materials and information in a professional manner
  • Excellent customer service skills and commitment to providing the highest level of customer satisfaction
  • Experience/familiarity with computers, business e-mail, communication systems and internet search capabilities; proficient in Microsoft Office products, specifically Word and Excel

Pay

  • $83,000 - $93,000 Depending on Experience
