Location: Fresno, CA. (Onsite)

Classification: Full-Time

This position is non-exempt and will be paid on an hourly basis.

Schedule:

Monday- Universal Healthcare MSO's standard operating hours are Monday through Friday, from 8:00 a.m. to 5:00 p.m. Pacific Time. Unless otherwise specified, all employees are expected to work their assigned schedules and remain available during these operational hours.

Compensation: The initial pay range for this position upon commencement of employment is projected to fall between $22.00 and $27.49. However, the offered base pay may be subject to adjustments based on various individualized factors, such as the candidate's relevant knowledge, skills, and experience. Position Summary:

The Community Outreach Coordinator is responsible for screening, education of services, outreach, and assisting with enrollment of members referred to Universal Healthcare MSO/ Sunrise Wellness ECM Program. Outreach efforts include telephonic and in-person outreach at the member’s home, in the community, clinic, or office. Assists with helping members and families navigate/access community programs and other resources. The staff is responsible for keeping detailed records of their efforts and communicating regularly with their direct supervisor. The position is responsible for reviewing qualified members who are referred to the ECM Program and for existing ECM members, who are unable to reach over the phone and/or mail. The staff identifies and develop relationships with strategic community partners (e.g., shelter, community partners, county agencies, health plans, and other government agencies) to help facilitate the enrollment and engagement of potential and existing ECM members.

Requirements:

Job Duties and Responsibilities:

•Conduct outreach and enrollment activities, including performing both in-person and on the phone outreach to eligible and existing members to promote program enrollment and/or retention.

•Reach monthly requirement quotas for ECM enrollment.

•Educate members on ECM Program benefits and services to promote program enrollment.

•Conduct activities to promote health services and wellness programs in-person, via telephone call, and other means of communication.

• Follow-up with members via phone calls, makes home visits and frequents other settings where members can be found.

• Conducts eligibility determination, enrollment and follow-up with members.

• Document all outreach activities and member encounters in Case Management Systems and collect required information to complete the enrollment process, including completion of screening forms and questionnaires.

• Participate in community outreach events, such as shelter visits, navigation clinic visits, and other events to perform member enrollment and engagement activities.

• Works closely and collaboratively with other ECM staff members.

• Maintain and handle confidential member information.

• Demonstrate strong verbal communication skills to leave a positive impression on every member and/or provider encounter.

• Assist in community outreach and marketing activities to increase program effectiveness in responding to the healthcare access needs for potential and existing members.

• Possess excellent communication skills to relate with diverse populations and community organizations.

• Use personal computers and related software applications (e.g., MS Word, Excel, and Case Management system)

• Provide follow-up on all pending applications to ensure the enrollment process is complete.

• Provide information in a manner that is culturally and linguistically appropriate for members.

• Maintain communication with supervisor and core team members as well as any other pertinent staff to meet program requirements and meet member needs.

• Participate in relevant planning discussions and meetings, including team and staff meetings.

• Track and collect data surrounding outreach and enrollment efforts to assure continuous enrollments.

• Maintain statistics/data on contacts; identify needs, problems, and service gaps.

• Track and tabulate contacts, application submission and post-enrollment follow-up.

• Attend appropriate community or network meetings to facilitate outreach and engagement activities in the community.

• Work with shelters, local schools, churches, community partners, and key stakeholders to train, coordinate and problem-solve so they can make appropriate referrals the ECM program.

• Support and contribute to effective safety and risk management efforts by adhering to established policies and procedures, maintaining a safe environment, promoting accident prevention, and identifying and reporting potential liabilities.

• Assist the ECM team to understand the culture, norms, beliefs and preferences of the members and their community by representing the voice of the community.

• Ensures documentation is accurate, useful and in compliance with regulatory requirements and accreditation standards.

• Participate in all appropriate staff meetings or other activities as needed.

• Perform other duties as assigned that support organizational goals and objectives.

Qualifications:

• Education: High School diploma or GED required.

• Experience: Community health or health care setting experience preferred

• Understanding of and sensitivity to mental health conditions and addictive disorders preferred

• Awareness of the impact of unmitigated bias and judgement on health

• Highly skilled interpersonally, with excellent teamwork and relationship skills

• Highly skilled in interpersonal communication, including resolving conflict.

• A high degree of skillful decision making and judgement, in an autonomous position, including knowing when to consult with the team, supervisors, and experts.

• Able to sufficiently engage members in a variety of settings, including on the phone, at member’s homes, in hospitals, clinics, shelters, and other settings.

• Possession of Community Health Workers (CHW) Certificate OR completion of CHW Certification within one year of acknowledging this job description.

• Demonstrated experience working with one or more of the ECM populations of focus, including but not limited to: adults and children experiencing homelessness, those with serious mental illness (SMI) or substance use disorders (SUD), high-utilizers of healthcare services, adults or children with complex physical, behavioral, or developmental conditions, individuals transitioning from incarceration, children/youth involved in child welfare, or those belonging to the birth equity population.

Knowledge and Skills:

• Familiarity with medical insurance, basic medical terminology, and a managed care environment.

• Bilingual proficiency in English and Spanish is preferred.

• Strong organizational skills with the ability to manage multiple priorities and projects effectively.

• General knowledge of Microsoft Office applications, including Excel, Word, and Outlook.

• Typing speed of 45 words per minute or more.

• Competency in using various office equipment such as desktop computers, copy machines, scanners, facsimile machines, and telephones, ensuring correct and safe usage.

• Capability to work autonomously with minimal supervision.

Other Requirements:

• Possession of valid driver’s license

• Proof of state-required auto liability insurance.

• Reliable transportation is required to perform essential duties, including traveling to locate and engage with members in the field or at various locations as needed.

Working Conditions:

This position may be subject to tuberculosis (TB) testing requirements based on job duties and work location. Employees whose roles involve regular contact with members will be required to provide proof of a current negative TB test prior to hire and periodically thereafter, in accordance with organizational policy and applicable public health regulations.

This position is performed in a professional office environment. Under normal working conditions, exposure to the following environmental conditions is not anticipated:
