Job Description
JOB SUMMARY:
Under the direction of the ECM Program Director, the Care Coordinator & Housing Navigator provides non-clinical care coordination and housing navigation services to members enrolled in California’s Enhanced Care Management (ECM) program. This position supports whole-person care by assisting members with healthcare navigation, connecting them to community and housing resources, and promoting engagement in their care plans.
The role works as part of a Community-Based Care Management Entity (CB-CME), collaborating closely with clinical team members to support care plan implementation, improve access to services, and address social determinants of health, including housing instability.
Requirements:MINIMUM & PREFERRED QUALIFICATIONS:
Education/Training
Minimum: High school diploma or equivalent.
Preferred: Medical Assistant (MA), Certified Nursing Assistant (CAN), Associate or Bachelor’s degree in a related field.
Experience
Minimum: Two (2) years of customer service or administrative experience.
Preferred: Experience in healthcare, managed care, or care coordination. Experience with referral processes, utilization management, or community-based services. Familiarity with ICD-10/CPT Coding.
Any combination of educational and work experience that would be equivalent to the stated minimum requirements would qualify for consideration of this position.
Certification(s)
Maintains a valid California Driver’s License and current automobile insurance when using a personal vehicle for Company business.

· Proficiency in Microsoft Office and general computer applications.
· Ability to work independently within a team-based environment.
· Bilingual (English/Spanish) preferred.
· Typing of 45 words per minute.
PHYSICAL, MENTAL & ENVIRONMENTAL REQUIREMENTS:
The physical, mental, and environmental demands described here are representative of those required to successfully perform the essential functions of this position. Primarily field-based role requiring regular travel between Company locations and other business sites as required; including standing, walking, and driving. Involves interacting with Members across home, clinic, and community settings, using standard technology, and managing multiple tasks independently in a dynamic environment.
PAY RANGE
$30.00 - $34.00 / hourly
