Job Description

Job Summary:

The Social Worker (LCSW) reports to the Rehab Therapy Manager of Rehabilitation Department.

MSW assists patients and their families with personal and environmental difficulties which interfere with obtaining maximum benefits from health and medical care. MSW uses professional knowledge and a team approach to provide medical and social interventions for patients. Provides discharge planning services, emotional care, advocacy, and maintains effective working relationships with community resources and services. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient’s status and interprets the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs, and to provide the care needed

The Social Worker (LCSW) should possess the ability to assess data, reflective of the patient's status, and interpret the appropriate information to identify and provide the needed care. This employee will access and use an electronic medical record for patient documentation and maintain confidentiality. This employee also will demonstrate knowledge and skills to appropriately communicate and interact with medical nursing staff and with patients, their families, and visitors while being sensitive to their cultural and religious beliefs. This employee will be responsible for all other assigned duties and tasks on an as-needed basis.

Performance Expectations:

Performance Expectations that are job specific will be covered during your department orientation. You will be assessed for your competency of these expectations during your first 90 days of employment

  • Demonstrates proficiency in required computer documentation of all assessments, communications, and interventions according to policy
  • Demonstrates the ability to use critical thinking with patient care evaluation and/or treatment
  • Assesses patient's capacity for self-care or support structure after discharge
  • Consults with patient, family members, and multidisciplinary team to determine the most appropriate discharge plan
  • Provides timely updates re: discharge planning to patient, individual acting on his or her behalf, and multidisciplinary team
  • Evaluates patient's need for post-hospital services, service availability, and arranges for post-hospital care before discharge to avoid unnecessary delays in discharge
  • Demonstrates knowledge of community resources and insurance coverage for both their scope of service and their limitations
  • Demonstrate the ability to provide services based on the theoretical and ethical frame work, of Social Work.
  • Demonstrate the ability to collaborate clinically with staff within various roles.
  • Demonstrate the willingness and ability to actively participate in helping achieve the objectives of the department; able to work independently as well as with a team.
  • Demonstrated the ability to complete all required trainings and/or certifications and attend all required meetings.
  • Demonstrate the ability to be flexible with changing priorities; able to tolerate high stress situations / mental demands and make decisions that are ethically and legally sound.

Qualifications:

Education/Skills

  • Master degree in social work from an accredited school of social work is required.
  • Two Years of Post Graduate Supervision completed and LCSW test completed and licensure approved by the Board of Social Work.
  • Must also possess proficiency in computer skills, oral and written communication, and interpersonal skills. .

Work Experience:

Two years of social work experience in a health-related agency or facility preferred.

Certification/Licensure-DUE UPON HIRE

  • Licensed Certified Social Worker (LCSW) as defined by Section 75-53-1 of MS Code of 1972

Mental Demands:

Must have a high energy level and be capable of handling high pressure situations, and demonstrate leadership and initiative in providing services. Ability to organize required tasks, cooperate with others, follow policy/procedure, and relate effectively with patients, families, physicians, and other health professionals, and the general public. Ability to initiate and guide others in problem-solving process. Ability to become familiar with community agencies.
