Position Purpose: The WTRC Intake Coordinator screens violently injured patients presenting to the Mount Sinai Hospital Emergency Department (ED) and inpatient trauma units for program eligibility into the Violence

Prevention & Recovery Services program. Working at the front end of the WTRC's care continuum, the Intake Coordinator is responsible for proactively identifying eligible patients — including those presenting with gunshot wounds, stab wounds, assaults, and domestic violence injuries — initiating compassionate, trauma-informed engagement, collecting essential psychosocial and demographic intake information, and facilitating timely, structured warm handoffs to the appropriate WTRC team members, including Social Workers, Violence Recovery Specialists (Trauma Recovery Specialists), Peer Support Workers, and Community Health Workers (CHWs). This role requires a high level of cultural competence, organizational acuity, and the ability to build immediate rapport with patients and families who are often in acute distress. The Intake Coordinator acts as the gateway to the WTRC's full service array and plays a critical role in ensuring that no violently injured patient leaves the hospital without a structured connection to recovery support. The Intake Coordinator works in close, ongoing partnership with ED Manager, Trauma Manager, social workers, and security personnel to embed WTRC services seamlessly into the ED care workflow.

This position reports to the Manager, Violence Prevention Initiatives.

*Ability to work rotating shifts, including evenings, weekends, and holidays, aligned with ED operations. *This position is classified as Essential Personnel and may be required to report to work during inclement weather or other emergency conditions.

Education and Work Experience:

High School Diploma or GED required. Associate's or Bachelor's degree in Social Work, Public Health, Psychology, Human Services, Healthcare Administration, or a related field preferred.

Equivalent combination of education and directly relevant professional experience will be considered.

Minimum two (2) years of experience in a healthcare, social services, case management, victim advocacy, or community health setting required. Experience working in or alongside an Emergency Department, trauma center, or other fast-paced clinical environment strongly preferred.

Prior experience with intake, screening, or enrollment processes in a health or social services program preferred.

Demonstrated experience working with trauma-affected populations, survivors of violence, or underserved urban communities required.

Lived experience as a survivor of violence or as a member of a community impacted by gun violence, with demonstrated professional stability and commitment to recovery preferred.

Prior experience in a Hospital-Based Violence Intervention Program (HVIP) or community violence intervention (CVI) setting preferred. Demonstrated experience executing warm handoffs across multidisciplinary teams in a clinical or social service environment preferred.

Experience with REDCap, ETO (Efforts to Outcomes), or similar case management and data platforms preferred. Knowledge and Skills:

Proficiency with electronic health record systems (Epic strongly preferred) and data entry platforms (REDCap or equivalent).

Strong written and verbal communication skills, with demonstrated ability to communicate clearly under pressure.

Bilingual proficiency in English and Spanish strongly preferred; other languages spoken on Chicago's West Side are a plus.

Familiarity with Chicago's West Side community landscape, service providers, and social service ecosystem preferred.

Knowledge of Illinois Crime Victim Compensation (ICVS), Medicaid enrollment, and other entitlement programs available to violence survivors preferred.
