POSITION PURPOSE |
The Licensed Practical Nurse (LPN) is responsible for preparing patients for examinations and delivering individualized patient care by promoting and restoring health through the nursing process. The LPN collaborates with physicians and multidisciplinary care team members to provide physical and emotional support to patients in a Primary Care setting. Responsibilities include assessing patient health concerns, administering nursing care, maintaining accurate medical records, and supporting continuity of care. The LPN will also contribute to the clinic’s efforts to achieve and maintain Patient-Centered Primary Care Home (PCPCH) recognition and support the effective use and optimization of electronic health records (EHR) and other health information technology systems. Additionally, the LPN will assist in implementing care plans, educating patients on health maintenance and disease prevention, and providing guidance and preventative care within the Scope of Practice for a Licensed Practical Nurse. |
