Registered Nurse (RN) – Home Health | Full-Time

Duluth, Norcross, Snellville, and Suwanee counties

$50–$140 Per Visit + Mileage Reimbursement (.55/mile) | Full-Time Benefits

What You'll Do

As a Home Health RN, you will provide skilled nursing care to patients in their homes while working closely with physicians, patients, families, and the interdisciplinary care team.

Your responsibilities will include:

  • Conducting comprehensive patient assessments and reassessments
  • Developing, implementing, and updating individualized Plans of Care
  • Providing skilled nursing interventions based on each patient's clinical needs
  • Completing accurate and timely OASIS assessments and clinical documentation
  • Educating patients and families regarding disease processes, medications, treatments, and plan of care
  • Monitoring patient progress and communicating changes in condition appropriately
  • Collaborating with physicians and the interdisciplinary care team
  • Promoting patient safety, quality outcomes, and continuity of care
  • Providing compassionate, professional care that allows patients to remain safely at home

Requirements:
  • Graduate of an accredited Registered Nurse educational program
  • Current Registered Nurse (RN) license within the state of practice
  • Home Health/Oasis experience required
  • Current CPR certification
  • Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
  • Strong initiative, follow through, process orientation, attention-to-detail
  • Reliable transportation, valid driver’s license and current automobile insurance
