Job Title: Disability Support Worker

Position Summary: We are seeking a dedicated and empathetic Disability Support Worker to join our team. The successful candidate will play a pivotal role in enhancing the quality of life for individuals with disabilities by providing personalized care and support in various settings, including clients' homes and community environments.
Key Responsibilities:
Personal Care Assistance: Assist clients with daily activities such as bathing, dressing, grooming, and mobility support.
Community Engagement: Support clients in participating in community activities, social events, and recreational pursuits to promote social inclusion.
Health Monitoring: Observe and report on clients' health and well-being, ensuring any changes are communicated to the appropriate healthcare professionals.
Skill Development: Assist clients in developing life skills, including cooking, budgeting, and household management.
Documentation: Maintain accurate records of care provided, incidents, and any changes in clients' conditions.
Collaboration: Work closely with a multidisciplinary team, including healthcare providers, family members, and other support staff, to develop and implement individualized care plans.
Qualifications:
Certificate III or IV in Disability Support or a related field.
Current First Aid and CPR certification.
Valid Working with Children Check and National Police Check.
Previous experience in disability support or a similar role is advantageous.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
