Key Responsibilities
Conduct 50–70+ detailed patient assessment calls to gather relevant health information and identify needs
Develop individualized care plans and support long-term health goals
Perform Behavioral Health Integration (BHI) assessments to evaluate mental health components of care
Provide clear, compassionate education to patients on their conditions, treatment plans, and self-care strategies
Identify and escalate patient needs such as medication refills, appointment scheduling, and referrals to community resources
Assist patients in troubleshooting medical devices and guide them through setup processes as needed
Respond to inbound calls, return voicemails, and conduct follow-up communication with patients
Help manage the scheduling and rescheduling of patient appointments
Maintain accurate documentation and ensure timely care interventions
Support and contribute to a positive and collaborative work environment that reflects Nsight Health’s values
Requirements
Active and unrestricted LVN or LPN license
Experience or strong passion for patient education and chronic care support
Ability to relate with empathy and provide compassionate, patient-first care
Quiet, HIPAA-compliant workspace
Reliable high-speed internet connection (speed test required during the interview)

Compensation & Benefits
Sign-On Bonus: This role is eligible for a one-time $3,000 sign-on bonus. The bonus will be paid after you complete 120 days of continuous employment and is contingent upon your active employment status and satisfactory performance at the time of payment
Bonus potential: Earn up to $1,500 per month

Pay Disclosure
The amount displayed is total potential compensation (OTE), which includes potential monthly bonuses, as we are a pay-for-performance company. Salary offers are determined based on the candidate’s qualifications, experience, skills, and education as they directly relate to the requirements of the position. Offers are also aligned with salary ranges informed by external market data for the role’s level.
Training Schedule
All new hires are required to complete a comprehensive training program:
Duration: Five weeks
Schedule: Monday through Friday, 9:00 AM to 6:00 PM Eastern Time
Note: Attendance is mandatory during this period to ensure full readiness before going live with patients.
