Job Description

GENERAL SUMMARY:

The Care Navigator (CN) supports and promotes the continuum of care for their patient population. The CN provides individualized assistance to patients, families and caregivers to promote optimal outcomes. This position works collaboratively with physicians, staff and other providers across the care continuum.

PRINCIPAL JOB FUNCTIONS:

  • *Commits to the mission, vision, beliefs and consistently demonstrates our core values.
  • *Collaborates with other healthcare professionals to develop a plan and facilitate referrals to address needs and barriers.
  • *Facilitates the coordination of patient care services to assure timeliness and excellence in patient care including coordination of services, treatment and communication among the medical team.
  • *Facilitates development, implementation and adherence to clinical and evidence-based guidelines, as appropriate, to ensure optimal clinical outcomes.
  • *Identifies and addresses barriers to timely and appropriate diagnosis and treatment, connects patients with the appropriate healthcare and community resources.
  • *Builds therapeutic and trusting relationships with patients, families and caregivers through effective communication and listening skills.
  • *Provides anticipatory guidance, education and psychosocial support throughout the care continuum.
  • *Communicates with patients throughout the continuum of care via face-to-face, telephonic, electronic, etc. as appropriate.
  • *Assists in achieving and maintaining recommended certification/accreditation..
  • *Maintains patient records accurately, completely, timely and confidentially according to procedures. Documents patient encounters and provided services.
  • *Serves as a resource and provides assistance to the care team
  • * Provides and/or collaborates with staff on patient and family education.
  • *Supports the continuity of care for optimal patient outcomes; performs telephone follow-up calls on patients post hospitalization and at regular intervals as appropriate.
  • *Assists in the identification and reporting of quality issues, and contributes to the performance improvement plan to assess and improve the quality and efficiency of the program including, but not limited to quality assessment, recommendations for program improvement and patient outcomes measures.
  • *Exhibits behaviors and actions which create a high level of patient/customer satisfaction, which contributes to positive patient/customer relations and which reflect respect for the patient's/customer's rights, needs and confidentiality.
  • *Ensures all patient care is provided in accordance with regulatory and safety requirements which may include Joint Commission, OSHA, state and federal regulations.
  • Assumes responsibility for professional growth and development by staying current in the field by reading related professional literature and undergoing professional educational efforts.
  • Performs other related duties as assigned.
  • Oncology Navigator Responsibilities

  • *Participates in weekly cancer conferences to further care planning and communication with healthcare providers.
  • Call Center RN Responsibilities

  • *Monitors responses from the automated call system, and makes timely personal calls for all responses that have triggered a potential additional patient need or question. Documents follow up within the appropriate document.
  • *Conducts serial post-discharge calls for specific patient populations (i.e. rehab, total joints) and completes specified assessments and documents within appropriate document.
  • *Resolves clinical, guidance and service issues with patients or escalates the issue as per protocol or identified algorithms.
  • *Creates / monitors data reports for assessing compliance with the established processes and meeting identified quality metrics.
  • *Serves as subject matter expert related to current technology utilized for post discharge call processes.
  • *Conducts consistent and timely outreach calls to patients that trigger an alert through our automated call system, that are identified as high risk for readmission or from other sources.
  • Clinic Care Navigator Responsibilities

  • *Works with designated patient populations and their families/caregivers to provide Self-Management Support by developing short and long-term goals, providing education and establishing the willingness to change, the importance of change and the confidence on the ability to change.
  • *Provide guidance and quality leadership through collaboration with BPN practices on strategies to achieve clinic level goals and improved patient outcomes.

    REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  • Maintains clinical competency as required for position including but not limited to age-specific competencies relative to patient’s growth and development needs and annual skill competency verification.
  • Knowledge of nursing theory and practice including leadership skills.
  • Knowledge of the principles of growth and development over the human life span.
  • Knowledge of computer hardware equipment and software applications relevant to work functions.
  • Ability to establish and maintain effective working relationships with health care team members, physicians and diverse patient populations.
  • Ability to analyze information, understand statistics and provide input into the development of policies and procedures.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to operate patient therapeutic equipment and/or machines as relevant to work duties.
  • Ability to balance and prioritize diverse management and clinical responsibilities.
  • Ability to assess data reflective of the patient’s status, interpret the needed to identify each patient’s requirements relative to their age-specific needs and provide necessary care.
  • Ability to maintain confidentiality of patient and employee information.
  • Ability to provide instruction and direction to others.
  • EDUCATION AND EXPERIENCE:

    Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. BSN required. Masters of Nursing preferred. Minimum of three (3) years recent clinical experience in area of assigned specialty (Oncology, Orthopedics, other).

    Care Navigator Clinic

    Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required. BSN preferred. Minimal of three (3) years recent clinical experience required and ambulatory clinic experience preferred.

    Other Credentials /Certifications

    Applicable Specialty Certification from an approved credentialing body in area of practice preferred and required within 18 months. Basic Life Support (CPR) certification required.

    Basic Life Support (CPR) certification required.

    PHYSICAL REQUIREMENTS:

    (Physical Requirements are based on federal criteria and assigned by Human Resources upon review of the Principal Job Functions.)

    (DOT) – Characterized as light work requiring exertion up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
