Caledonia Home Health Care & Hospice is seeking a Physical Therapist up to 40 hours/week. The successful candidate will provide in-home therapy services to maximize the independence and safety of our home care and hospice patients. Must work well independently while also being a collaborative member of our multidisciplinary team. Requires VT licensure as a Physical Therapist, a valid driver’s license and reliable vehicle.
