About the Role:
Primary Care Partners is seeking a compassionate and reliable LPN to join our team. In this role, you will work directly with patients and providers to ensure exceptional care in a collaborative clinic setting. This is a versatile position that includes patient rooming, triage, phone-based care coordination, and direct support to providers.

Responsibilities include:

  • Assisting physicians with patient examinations and procedures
  • Recording patient medical histories and vital signs accurately
  • Administering medications, injections, and vaccines
  • Performing phlebotomy and basic lab testing
  • Preparing and maintaining clean exam rooms and medical equipment
  • Managing nurse-directed patient telephone calls and triage
  • Providing timely medical advice, test results, and patient education
  • Collaborating with providers on urgent patient needs and care decisions
  • Helping manage prescription refills per protocol
  • Supporting overall clinic flow and teamwork
  • Ensuring accurate documentation and continuity of care

Qualifications:

  • Completion of an accredited Nursing program
  • Current LPN license (Required)
  • Previous clinical or medical office experience preferred
  • Proficiency in electronic health record systems
  • Strong clinical judgment, critical thinking, and organizational skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced, team-oriented environment

Job Type: Full-time (36 hours/week)
