About the Role:
Primary Care Partners is seeking a compassionate and reliable LPN to join our team. In this role, you will work directly with patients and providers to ensure exceptional care in a collaborative clinic setting. This is a versatile position that includes patient rooming, triage, phone-based care coordination, and direct support to providers.
Responsibilities include:
- Assisting physicians with patient examinations and procedures
- Recording patient medical histories and vital signs accurately
- Administering medications, injections, and vaccines
- Performing phlebotomy and basic lab testing
- Preparing and maintaining clean exam rooms and medical equipment
- Managing nurse-directed patient telephone calls and triage
- Providing timely medical advice, test results, and patient education
- Collaborating with providers on urgent patient needs and care decisions
- Helping manage prescription refills per protocol
- Supporting overall clinic flow and teamwork
- Ensuring accurate documentation and continuity of care
Qualifications:
- Completion of an accredited Nursing program
- Current LPN license (Required)
- Previous clinical or medical office experience preferred
- Proficiency in electronic health record systems
- Strong clinical judgment, critical thinking, and organizational skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced, team-oriented environment
Job Type: Full-time (36 hours/week)
