About This Position
Key Responsibilities
Deliver services in accordance with the client’s individualized plan of care.
Conduct initial evaluations and regularly reassess client needs.
Develop, implement, and revise care plans as required.
Initiate appropriate preventive and rehabilitative non-medical procedures.
Document client assessments, care plans, supervisory visits, and caregiver/client education.
Coordinate care services and communicate changes in client condition to appropriate personnel.
Adhere to established policies and procedures, ensuring compliance with regulatory standards.
Implement and oversee the nursing care plan for each client.
Provide non-medical treatments and preventive procedures as assigned.
Monitor and report client responses to care, including physical or emotional changes, to the care coordinator or physician as appropriate.
Educate and counsel clients and their families on their care needs and resources available.
Maintain accurate and complete records, including observations, non-medical treatments, and care provided.
Participate in medical record audits and attend scheduled staff meetings, patient care conferences, and in-service training.
Take on-call duties during nights, weekends, and holidays as assigned.
Submit schedule changes, updates, and required documentation promptly.
