About the Role:
The Director of Quality (RN) is responsible for supporting clinical quality, documentation accuracy, and regulatory compliance in the home health setting. This role reviews patient records, OASIS assessments, care plans, and visit documentation to ensure alignment with agency policies, physician orders, Medicare/CMS guidelines, and applicable state and accrediting standards. The Director of Quality partners with clinicians and leadership to identify documentation issues, reduce audit risk, improve quality outcomes, and promote timely, accurate chart completion.
Essential Job Functions/Responsibilities
- Review clinical records, visit notes, care plans, and OASIS assessments for accuracy, completeness, timeliness, and internal consistency.
- Ensure documentation supports physician orders, homebound status, skilled need, and other coverage requirements for home health services.
- Audit charts for compliance with Medicare/CMS requirements, Conditions of Participation, state regulations, payer guidelines, and agency policies.
- Identify documentation deficiencies, coding or sequencing issues, missing signatures, unsigned orders, and other compliance risks; communicate findings for correction.
- Collaborate with clinicians, intake, scheduling, coding, and leadership teams to resolve documentation issues before billing or submission.
- Support quality improvement initiatives related to documentation quality, patient outcomes, timeliness, and survey readiness.
- Provide education and feedback to field staff and office staff regarding documentation standards, OASIS accuracy, and regulatory updates.
- Assist with preparation for internal audits, external surveys, payer reviews, and accreditation activities.
- Maintain productivity standards, review turnaround times, and confidentiality of protected health information.
Position Qualifications
- Current, active Registered Nurse (RN) license in the applicable state of practice.
- Minimum of 2 years of nursing experience, including home health experience.
- Working knowledge of Medicare/CMS home health regulations, Conditions of Participation, and documentation standards.
- Experience reviewing clinical documentation, auditing charts, or supporting compliance and quality activities.
- Strong understanding of OASIS guidance, care planning, and interdisciplinary coordination in home health.
- Excellent written and verbal communication skills with strong attention to detail.
- Proficiency with electronic medical records and standard office software.

The Director of Quality utilizes their expertise in quality management principles and regulatory standards daily to design and refine quality systems that ensure compliance and operational efficiency. Strong leadership and communication skills are essential for guiding diverse teams, facilitating collaboration across departments, and presenting quality insights to senior management. Analytical skills are applied to interpret complex data sets and identify root causes of quality issues, enabling targeted improvements. Proficiency in quality tools such as Six Sigma and statistical software supports data-driven decision-making and process optimization. Additionally, negotiation and stakeholder management skills are critical when working with regulatory bodies, suppliers, and customers to uphold quality commitments and resolve concerns.
