This role focuses on providing direct outreach and care coordination support to patients with complex medical and social needs, helping them better navigate the healthcare system and access essential services. You will work closely with multidisciplinary clinical teams to support care plans, close gaps in care, and improve overall patient outcomes. The position blends community-based engagement with remote coordination, including phone outreach, home visits, and accompaniment to medical appointments when needed. You will also play an active role in patient education, medication adherence support, and safety assessments in home environments. This is a highly collaborative role where communication, empathy, and responsiveness directly influence patient well-being. It is well suited for someone who is passionate about community health and making care more accessible and effective for vulnerable populations.
- Conduct outreach to patients with complex medical and social needs and support their engagement with care services
- Assist in identifying care gaps and connect patients with appropriate healthcare and community resources
- Collaborate with multidisciplinary teams to support the development and execution of individualized care plans
- Support patients through phone calls, home visits, hospital visits, and accompaniment to medical appointments
- Facilitate telehealth visits and ensure patients can effectively engage with providers
- Provide disease-specific education and reinforce treatment plans under clinical guidance
- Conduct medication reviews to support adherence and identify potential issues
- Perform basic clinical and safety assessments, including vital signs and home safety evaluations
- Communicate patient concerns to clinical teams and participate in care coordination meetings
- Support training and orientation of new Community Health Workers when applicable
- High school diploma or GED required
- Minimum 1 year of relevant experience in healthcare, community outreach, or social services
- Basic Life Support (BLS) certification required
- Valid driver’s license required
- Strong communication skills with the ability to engage patients from diverse backgrounds
- Ability to multitask, problem-solve, and work effectively in a team-based environment
- Comfort working independently in community and home-based settings
- Strong organizational skills and attention to detail
- Compassionate, patient-centered approach with strong interpersonal skills
- Basic clerical and documentation skills, with proficiency in standard communication tools
