Job Description

The Holyoke Visiting Nurse Association, Inc., established in 1905, is dedicated to providing quality skilled care to individuals in the comfort of their home. We coordinate and deliver comprehensive, cost-effective services, including nursing, physical therapy, occupational therapy, speech therapy, social work and home health aide care. In addition, we are committed to promoting community health through education and special programs.

Position Summary:

The Clinical Liaison/General Inpatient Specialist’s responsibilities include the following;

Champion and promote the mission and vision of the organization and project a positive and professional image for the Agency in the community.
Enhance and promote the entire Agency, including hospice, in the marketplace by regularly meeting with, communicating with and providing information and education to professionals, facilities, organizations, and consumers regarding services provided as well as coverage criteria and reimbursement for these services.

Collaborate with referral sources, Agency staff, physicians, and other professionals to ensure that patient services are developed and delivered in a safe, effective manner to assure that customer satisfaction is exceptional.

Maintain an in-depth knowledge of the community, health care facilities, and referral sources appropriate for agency services.

Assist the VNA/Hospice leadership team in establishing targets and goals for development of all service lines in the market.

Participate in assigned committee activities.

Will adhere to the agency policies, which includes the Corporate Responsibility program.

Communicates with customers in a competent, respectful manner to encourage positive attitudes about the agency.

Assume a regular plan in the supervisory on-call schedule as assigned or requested.

Assists and supports other departments when needed.

Perform other duties as assigned and/or requested.

Minimum Requirements:

  • Bachelor’s in nursing (BSN) preferred
  • Current MA RN License required
  • One (1) – three (3) years of Hospice & Home Health Care required
