JOB TITLE: Pharmacist

REPORTS TO: CHIEF NURSING OFFICER

Job Summary:

Support hospital pharmacy operations to ensure provider orders are reviewed and processed, and medications are dispensed to patients upon receipt, in as safe a manner as possible. Comply with all Hospital and Comprehensive Pharmacy Services (CPS) policies as well as state and federal laws when working in the pharmacy.

Essential Duties and Responsibilities:

Evaluates physician drug orders for appropriateness with regard to patient condition, organ function, disease state, and concomitant therapies to promote optimal patient outcomes. Includes, collection and interpretation of information regarding patients’ allergies, organ function, drug history, and other information pertinent to drug therapy.

Processes provider medication orders including, but not limited to, order entry into dispensing computer system, appropriate product selection, compounding, labeling and repackaging with completion of appropriate records; delegating product preparation activities to pharmacy technicians when appropriate, within legal limits and hospital policies and procedures.

Consults with various health care professionals and provides information and guidance concerning drugs and their appropriate use.

Ensures drugs are reasonably available to patients when needed to avoid delays in therapy and promote positive patient outcomes.

Comply with departmental and hospital policies and procedures.

Assumes duties of Director of Pharmacy in his/her absence to manage daily departmental activities

Assists in orienting new personnel or students to departmental services and programs.

Responds to cardiopulmonary resuscitation procedures, when requested.

Assists in ensuring drug storage locations are maintained appropriately (e.g., floor stock, automated dispensing systems, surgery carts, emergency carts, "crash" carts, etc.).

Assists in ensuring that drug items are appropriately billed to various areas or patients.

Assists in ensuring that pharmacy inventory is maintained at appropriate levels.

Procures formulary and non-formulary items to provide optimal drug therapy.

Reviews and assumes responsibility for products prepared by pharmacy technicians and assigns tasks to pharmacy technicians as necessary to meet workload demands.

Participates in and/or provides support to hospital committees as agreed upon by committee members and the Director of Pharmacy to help establish drug-related policies, criteria, or to monitor issues related to drugs i.e. Pharmacy and Therapeutics Committee.

Assist in the preparation of periodic reports on the progress of the department for submission to the administration of the hospital.

Maintains State and Federal required records so as to maintain adequate accountability and control.

Participate in departmental and hospital performance improvement activities to promote quality in the medication use process, including documentation of pharmacist interventions.

Assists the department and hospital in maintaining compliance with accreditation standards.

Maintains knowledge of state and federal pharmacy and applicable health care regulations and professional standards as they relate to this job description.

Adheres to requirements of state and federal pharmacy laws and regulations in the performance of duties.

Adheres to applicable hospital policies and procedures.

Assist in presenting medical, nursing, or department in-service programs.

Provide patient education, as needed, on medications.

Other duties as assigned.

Through training and/or experience, possess necessary knowledge to provide oversight of medication regimens for the patient populations treated in the facility including a knowledge of dosing for specific age populations.

Professional Requirements:

Must exercise sound administrative and professional judgment using initiative consistent with a high level of patient care.

Requires a very high degree of accuracy. Is responsible for the safety of patients and department personnel under his or her supervision.

Adheres to state and federal laws, regulations, standards, company policies and procedures, and Code of Conduct.

Maintains the confidentiality of patient, hospital, and corporate information.

Requires the ability to work efficiently under broad guidelines and must frequently exercise his or her own judgment in decision making.

Effective communication skills - verbal and written.

Requirements:

Education and/or Experience:

Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) with a minimum of a bachelor’s degree.

Doctor of Pharmacy preferred.

Some Hospital Pharmacy work experience preferred.

Post-graduate Hospital pharmacy residency preferred.

Proficient computer skills using Pharmacy systems and Microsoft applications – Outlook, Word, Excel.

Certificates and Licenses:

Licensure in good standing as a Registered Pharmacist in the state of practice.
