  • JOB SUMMARY AND SPECIFICATIONS
    • JOB SUMMARY
      • The RN Care Coordinator facilitates comprehensive care for patients with cardiometabolic disease by conducting assessments, providing education, supporting self-management, and ensuring timely healthcare delivery to achieve optimal health, quality, and financial outcomes. In addition to care coordination, this role performs nursing triage duties, evaluating patient needs, routinely assessing the patient’s clinical status, and prioritizing care based on clinical urgency. This involves working closely and collaboratively with all members of the interdisciplinary care team. The RN Care Coordinator contributes to improved quality of care through efficient resource utilization and acts as an advocate for patients, helping to minimize fragmentation in healthcare delivery. This position is dedicated to the ongoing pursuit of excellence in improving the health status of the community.
    • MINIMUM QUALIFICATIONS
      • Professional & Technical Skills
        • Case Management, Care Coordination, and/or Nurse Navigation experience preferred.
      • Education
        • Graduate of an accredited School of Nursing.
          BSN preferred or obtained within 6 years of employment
      • License(s) or Certification(s)
          • Current Indiana license as a Registered Nurse, BSN or obtain within 6 years of employment. (Current ASNs with MHP refer to SPP No. HR-18)
      • Other Skills or Requirements
          • Excellent written, verbal, listening and communication skills. Communicate appropriately and clearly to staff and providers.
          • Promotes positive interpersonal relationships among patients, providers, and staff.
          • Ability to work well independently, while collaborating with other team members. Serves as a clinical resource person to providers and staff.
          • Ability and willingness to self-motivate, to prioritize and change processes to improve effectiveness and efficiency. Adapts to changing patient and/or organization's priorities.
