Job Description
Community Medical Group is looking for a Referral Coordinator to help ensure our patients receive the specialty care and services they need.
In this role, you’ll help coordinate referrals, authorizations, appointments, and follow up while serving as an important connection between our patients, medical centers, specialists, and other healthcare providers.

What You’ll Do• Coordinate and process patient referrals for specialty services.
• Verify patient demographics, insurance information, and eligibility to ensure information is accurate and up to date.
• Obtain and process required authorizations for specialty services.
• Schedule specialist appointments, testing, and transportation when needed.
• Communicate with patients regarding referral status, appointments, and next steps.
• Follow up with patients to help ensure they complete recommended referrals and services.
• Work closely with physicians, medical centers, specialists, and other healthcare providers to coordinate patient care.
• Enter referrals, authorizations, hospital services, outpatient services, and other specialty care information into the appropriate systems.
• Monitor outstanding referral reports and follow up on missing information in a timely manner.
• Maintain accurate documentation while protecting patient confidentiality and following HIPAA requirements.
• Support the team with additional referral related administrative responsibilities as needed.
What We’re Looking For• High school diploma or equivalent required.
• Previous experience in a clinical or healthcare setting.
• Strong computer skills, including experience with Microsoft Office.
• Strong organizational skills with the ability to manage multiple priorities.
• Excellent attention to detail and accuracy.
• Strong written and verbal communication skills.
• Comfortable communicating with patients, providers, and members of the healthcare team.
• Ability to work independently while also contributing to a team environment.
• Strong problem solving skills and the ability to use good judgment.
• Knowledge of medical terminology preferred.
• Experience with Electronic Health Records preferred.
• Knowledge of CPT, HCPCS, and ICD coding preferred.
• Medical Assistant training or certification preferred.
• Bilingual English/Spanish or English/Creole preferred.
