Job Description: PRN Physical Therapist Assistant (PTA) – Home Health
The PRN Physical Therapist Assistant (PTA) provides skilled physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist (PT) and in accordance with physician orders, the established plan of care, agency policies, and applicable state and federal regulations. The PTA assists patients in improving mobility, strength, balance, and functional independence while promoting safety and quality of life.
Essential Duties and Responsibilities
· Provide skilled physical therapy treatments as outlined in the physical therapist's plan of care.
· Implement therapeutic exercises, gait training, transfer training, balance activities, endurance training, and functional mobility interventions.
· Monitor patient response to treatment and promptly report changes in condition or progress to the supervising Physical Therapist.
· Reinforce patient and caregiver education regarding home exercise programs, safety techniques, use of assistive devices, and fall prevention.
· Assist patients in maximizing functional independence and achieving established rehabilitation goals.
· Observe and document patient progress, treatment interventions, and responses in accordance with agency policies, Medicare guidelines, and regulatory requirements.
· Communicate effectively with the supervising Physical Therapist regarding patient status, treatment effectiveness, and recommendations for plan of care modifications.
· Participate in interdisciplinary team meetings and case conferences as requested.
· Complete all required documentation accurately and within established timeframes.
· Maintain compliance with Medicare Conditions of Participation, HIPAA regulations, infection control standards, and agency policies.
· Maintain professional standards of practice and continuing competency.
· Accept PRN patient assignments based on agency staffing needs and availability.
Qualifications
· Graduate of an accredited Physical Therapist Assistant program.
· Current, unrestricted Physical Therapist Assistant license in the state of practice.
· Current CPR/BLS certification.
· Valid driver's license, reliable transportation, and current automobile insurance.
· Minimum of one year of clinical experience preferred; home health experience preferred.
· Knowledge of Medicare home health regulations and documentation requirements is preferred.
· Strong interpersonal, organizational, and communication skills.
· Ability to work independently while maintaining regular communication with the supervising Physical Therapist.
Physical Requirements
· Ability to travel independently to patients' homes throughout the assigned service area.
· Ability to lift, transfer, and assist patients as needed (up to 50 pounds with or without reasonable accommodation).
· Ability to stand, walk, bend, kneel, stoop, and perform physical activities required to provide patient care.
· Ability to safely operate a motor vehicle and transport therapy equipment as needed.
Supervision
The Physical Therapist Assistant practices under the supervision of a licensed Physical Therapist and provides services only as permitted by state practice acts, Medicare regulations, and agency policies. The PTA does not perform initial evaluations, establish or modify plans of care, or complete discharge assessments unless permitted by applicable law and regulation.
Work Schedule
This is a PRN (as-needed) position. Hours are not guaranteed and are based on patient census and agency staffing needs. The PRN Physical Therapist Assistant is expected to maintain flexibility and communicate availability for patient assignments.
