Manager of Care Management in Windsor, Vermont at Mount Ascutney Hospital

Position Summary

This role is responsible for assuring the receipt of high quality, cost-efficient care for all patients who seek services at Valley Regional Hospital; the oversight of social work, case management, utilization review and discharge planning; developing expected patient outcomes, defining appropriate resource utilization and works collaboratively with providers to set goals; assuring the department’s work an hospital compliance is in line with federal and state regulations, reimbursement guidelines as well as the policies and procedures of Valley Regional Hospital.

Qualifications

Completion of a Master’s Degree Program in Social Work or is a graduate of an accredited school of nursing.

Licensure & Certifications

Current RN licensure in the State of New Hampshire (for RN applicants). Minimum of 5–7 years of experience in the hospital setting as a case manager and a member of an interdisciplinary team.
