Dates of Coverage: March 16- May 29

Hours: M & F - 7a-4p, T & TH - 8a-5p & W - 10a-7p

DOT & DEA required for this coverage

Basic primary care to employees

Occ health provided

$90 per hour

Travel/lodging covered, if needed

Must have NY licensure

2+ years experience in primary care as an FNP
