Key Responsibilities:

• Conduct thorough clinical evaluations and assessments of residents’ functional abilities and needs under physician referral.
• Develop, implement, and progress individualized, evidence-based treatment plans tailored to geriatric rehabilitation.
• Document patient progress accurately and efficiently in accordance with facility policies and regulatory standards.
• Collaborate fluidly with physicians, nursing staff, and rehab team members to ensure holistic resident care.
• Educate residents and their families on safe mobility strategies and home exercise programs.

Positions available in our Templeton, San Luis Obispo and Morro Bay facilities.

Position Requirements:

• Licensure: Current, active license to practice Physical Therapy in the State of California (Required).
• Physical Standards: Ability to safely lift up to 50 pounds and comfortably navigate standard clinical demands.
• Backgrounds: Experience in skilled nursing (SNF) or geriatric rehab is highly preferred, but passionate new graduates or setting-transitioners are welcome!
• Clearances: Ability to successfully pass standard background, physical, and TB checks.
