Job Description

Description:

About the Role
Touchstone Health is seeking a full-time Hospice Social Worker in San Antonio who is called to walk alongside patients and families during some of life’s most meaningful and vulnerable moments. In this role, you’ll provide compassionate, high-quality psychosocial care that supports not just clinical outcomes—but dignity, understanding, and peace.

You’ll primarily serve hospice patients, while also supporting home health patients as needed, offering a broader scope of impact across the continuum of care. You’ll be part of a collaborative team grounded in trust, empathy, and shared purpose. Here, your work goes beyond coordination—you help bring clarity to complex situations, advocate for patient needs, and bring out the courage in others when it matters most.

Key Responsibilities

  • Provide social work services to hospice patients in accordance with the physician’s plan of care
  • Support home health patients as needed, ensuring continuity of psychosocial care across settings
  • Support patients and families in navigating emotional, social, and environmental challenges
  • Connect patients to community resources, financial assistance, and support services
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Assist with discharge planning and care transitions to ensure continuity and safety
  • Document assessments, interventions, and care coordination accurately and in a timely manner
Requirements:

What We’re Looking For

  • Master’s degree in Social Work (MSW) from a CSWE-accredited program
  • Active licensure as required by the state of Texas
  • At least one year of social work experience in a healthcare setting (hospice experience preferred)
  • Strong skills in communication, assessment, and patient advocacy
  • Deep empathy and the ability to navigate complex family dynamics with sensitivity and professionalism
  • Detail-oriented, self-directed, and comfortable working independently in a field-based role

Why Join Us
At Touchstone Health, you’re not just stepping into a role—you’re joining a team shaped by purpose and guided by values. Our mission is simple but powerful: to bring out the courage in others.

We live this out by supporting one another, showing up with integrity, and creating space for growth—both professionally and personally.

Here’s what you can expect:

  • Supportive leadership that invests in your development and success
  • Growth opportunities within a mission-driven healthcare organization
  • Flexibility in your schedule, with autonomy in how you manage your day
  • A collaborative team culture where your voice matters and your work is valued
  • The opportunity to make a meaningful impact across both hospice and home health settings

This is a role for someone who is warm, grounded, and attentive to detail—someone who can sit with complexity, listen deeply, and advocate with clarity and compassion.
