Location: Pomonok Community Center – Queens, NY
Status: Full-Time
Schedule: This is a full-time position with standard hours of 9:00 AM to 5:00 PM. The role requires a regular on-site presence in the Pomonok community and occasional evening or weekend availability to support community activities.

Position OverviewQueens Community House is seeking a Family Support Coordinator (FSC) to serve as a key member of the Pomonok Community Center team, contributing to the Pomonok initiative’s strategic objectives. The FSC builds meaningful relationships with residents and supports households as they work toward greater stability.
As an engaged and accessible presence within the Pomonok community, the FSC actively listens to residents’ concerns and serves as a trusted connector between residents and the services, resources, and opportunities available through the Pomonok Community Center and Queens Community House.
Responsibilities include assessing requests from community members, working collaboratively with families to develop action plans, and connecting households to benefits screening, housing assistance, mental health services, financial counseling, and other critical resources.
In partnership with QCH Youth Services staff at PS 201, the FSC also supports engagement with parents and caregivers, helping connect families to resources and strengthening collaboration between the school, families, and community services.
Key Responsibilities- Build and maintain strong, trusting relationships with residents, serving as a community-facing connector to services, resources, and opportunities through the Pomonok Community Center, PS 201, and QCH programs.
- Conduct comprehensive household assessments and provide integrated case management using a strength-based, family-centered approach.
- Work with participants to identify priorities, explore needs, and develop actionable plans that support household stability and well-being.
- Connect individuals and families to Queens Community House programs and external resources, including:
- Benefits enrollment
- Housing assistance
- Mental health services
- Financial counseling
- Education and workforce programs
- Other community-based supports
- Partner with QCH Youth Services staff at PS 201 to engage parents and caregivers and strengthen collaboration between schools, families, and community services.
- Support the Program Director with community engagement, relationship-building, and advocacy, fostering partnerships across QCH sites and school-based programs.
- Participate in cross-department meetings and case conferencing to ensure coordinated support across programs and services.
- Maintain accurate participant records and apply a data-driven approach to track engagement, milestones, and outcomes.
- Contribute to creating a welcoming and supportive environment where residents can access resources, build connections, and improve their quality of life.
- MSW preferred with 2–3 years of experience; OR
- Bachelor’s degree in social work, human services, or related field with 5+ years of case management and community engagement experience.
- Demonstrated strength in conducting assessments, navigating complex conversations, identifying deeper needs, and prioritizing action steps.
- Strong collaboration skills and ability to work effectively with families, schools, community partners, and internal teams.
- Ability to work independently with high autonomy while maintaining accountability for outcomes and program objectives.
- Excellent communication and interpersonal skills, with the ability to engage diverse community members.
- Strong organizational skills and attention to detail, with the ability to manage multiple cases simultaneously.
- Knowledge of community resources, housing programs, and public assistance systems preferred.
- Experience using a data-driven approach to track participant engagement and outcomes; familiarity with Salesforce or similar database systems preferred.
