Job Description

Position Summary:

We’re seeking a detail-oriented and compassionate Home Care Program Supervisor to oversee Consumer intakes and assessments, match and monitor Personal Care Assistants (PCAs), resolve conflicts, and ensure compliance with AHCCCS and Electronic Visit Verification (EVV) requirements. This is a full time position at our Glendale office.

Program Service:

Ability360 delivers high-quality, person-centered non-medical home care services including attendant care, personal care, homemaker, and respite to AHCCCS/ALTCS/DDD eligible members in their homes or places of residence.

Key Responsibilities:

  • Conduct Consumer assessments and supervisory on-site visits.
  • Assign PCAs based on skill and Consumer needs.
  • Monitor PCA performance and provide training.
  • Maintain accurate documentation and communication with case managers.
  • Support conflict resolution between Consumers and PCAs

Qualifications:

  • 3+ years’ combined education/experience in case management or home care supervision.
  • Knowledge of EVV systems and AHCCCS health plans.
  • Bachelor’s Degree preferred.
  • Valid Fingerprint Clearance Card, driver’s license, clean driving record, and reliable transportation.
  • Bilingual a plus.

Schedule: Monday–Friday, 8:00 AM – 5:00 PM, with occasional evenings/weekends.
