Pharmacist Position Summary

The Ambulatory Pharmacist is responsible for the day-to-day delivery of high-quality, patient-centered pharmacy services within TidalHealth Community Pharmacy. In compliance with all federal and state laws, regulatory requirements, accreditation standards, and organizational policies, the pharmacist ensures the safe, accurate, and efficient dispensing of medications while promoting positive patient outcomes and an exceptional patient experience.The pharmacist serves as a trusted medication expert and integral member of the healthcare team. Through clinical expertise, patient advocacy, collaboration, and operational leadership, the Ambulatory Pharmacist plays a critical role in advancing TidalHealth's mission by improving access to care, optimizing medication outcomes, and delivering exceptional pharmacy services throughout the communities we serve.

Key responsibilities include:

1. Providing comprehensive pharmaceutical care through prescription verification, prospective drug utilization review, patient counseling, immunization services, medication therapy management (MTM), medication synchronization, and adherence programs designed to improve medication safety and therapeutic outcomes.
2. Collaborating with TidalHealth physicians, advanced practice providers, nurses, care coordinators, and other healthcare professionals to resolve medication-related issues, facilitate transitions of care, improve medication access, and support population health initiatives.
3. Supporting patient access and continuity of care through prior authorization assistance, medication coordination, adherence packaging, home delivery services, formulary management, and cost-savings opportunities.
4. Ensuring compliance with pharmacy operations, inventory management, controlled substance requirements, payer regulations, quality assurance programs, and all applicable regulatory and accreditation standards.
5. Providing leadership, training, and professional oversight to pharmacy technicians and support staff while fostering a culture of safety, accountability, teamwork, service excellence, and continuous improvement.
6. Contributing to organizational goals through clinical programs, operational initiatives, community outreach, workflow optimization, and business growth strategies that enhance the value of pharmacy services for patients and providers.

Pharmacist Position Requirements

Education

• B.S. Pharmacy or PharmD. Graduate of an accredited school of pharmacy
• Current pharmacist licensure in the state in which they will work and licensure in both Delaware and Maryland within 6 months of hire.

Experience

• None required.

• PGY-1 Residency preferred.

Work Schedule

• Rotational coverage of weekends.
• Expected to work overtime and/or additional weekends in times of staffing shortages, emergencies and unexpected increases in workload.
