Position Description – What the Role Entails:
The AOD INTACT Case Manager provides intensive, home-based case management and support services to families referred by DCFS where a parent or caregiver is struggling with Alcohol and Other Drug (AOD) use. The primary goal is to ensure the safety and well-being of the children, strengthen family function, facilitate the parent's engagement and success in AOD treatment and recovery, and prevent the removal of children from the home.

Areas of Accountability - What you will be doing:
Case Management and Services Coordination (AOD and Family Focus):
- Conduct comprehensive, strength-based assessments (including substance abuse, parenting capacity, safety, mental health, and environmental factors) to identify individual and family needs.
- Develop, implement, and monitor individualized service/case plans in collaboration with the family, integrating substance abuse treatment goals, recovery support, and child safety planning.
- Provide intensive, regularly scheduled home visits (often weekly) to monitor the family environment, assess child safety, and provide in-home support.
- Coordinate and link families to a wide range of community resources, including AOD treatment (inpatient, outpatient, detox), mental health services, medical care, housing assistance, employment/education support, and parenting programs.
- Accompany clients to appointments and meetings, and provide transportation assistance as needed, to reduce barriers to service engagement.
- Facilitate communication and collaboration between the family, AOD treatment providers, Child Welfare staff, the court system (if applicable), and other service agencies.
- Provide crisis intervention and support as needed to ensure immediate child safety and family stability.
- Monitor progress in AOD recovery using tools like the Recovery Matrix and adjust the case plan accordingly.
- Educate and advocate for the family within the treatment and child welfare systems.
Documentation and Compliance:
- Maintain timely, accurate, and thorough case notes and documentation in accordance with agency, state/local Child Welfare, and funder regulations (e.g., in a system like SACWIS, HMIS, or EHR).
- Prepare and submit all required reports, including court reports, progress summaries, and statistical data.
- Adhere strictly to confidentiality and HIPAA standards, as well as mandated reporting laws for child abuse and neglect.
Teamwork and Professionalism:
- Actively participate in clinical supervision, team meetings, case conferences, and AOD/Child Welfare trainings.
- Build and maintain positive, collaborative, and professional working relationships with families, colleagues, and external partners.
- Demonstrate cultural sensitivity and a trauma-informed approach in all interactions.

Rincon’s Work Environment – What to expect:
RSS will be expected to be ambulatory, but accommodations will be made as needed. Employees are responsible for informing supervisors of any invisible disability or medical restriction that limits physical capability. Light to moderate lifting.
Work environment: There may be occasions that call for an employee to carry out duties at other agency offices.
Schedule:
RSS schedules may change according to the task at hand. However, the base schedule is from 9:00am to 5:00pm Monday through Friday. Community events can be scheduled at the weekends and so the staff will arrange with the supervisor to how to adjust the schedule to meet the minimum 35-hour work week.

Requirements:The Knowledge and Expertise that Matters Most - What you bring to the Role:
- Comprehensive knowledge of AOD abuse, addiction, and the recovery process, including evidence-based treatment modalities and 12-step philosophy.
- Knowledge of the impact of substance abuse on child development, parenting, and family dynamics.
- Familiarity with the local network of community resources, especially AOD treatment and social services.
- Exceptional crisis intervention, de-escalation, and problem-solving skills.
- Strong organizational skills and the ability to manage a demanding caseload.
- Excellent written and verbal communication skills.
- Ability to work independently, manage time effectively, and maintain professional boundaries.
- Proficiency in computer applications, including case management software, electronic health records (EHR), and Microsoft Office.
Position Qualifications – What is required to Perform the Role:
- Required: Bachelor’s degree in social work, Psychology, Counseling, Human Services, or a closely related field.
- Preferred: Master's Degree in a related field.
- Required: Minimum of 1-2 years of direct experience in case management, social work, or a behavioral health setting, with a strong focus on substance abuse and/or child welfare/family services.
- Experience with the target population (families with AOD and child welfare involvement) is highly preferred.
- Licensing/Certification:
- Required: Certification or Registration as an Alcohol and Drug Counselor (e.g., CADC, CCAPP, CADTP, or equivalent in the state of employment) or ability to obtain within a specified time frame (e.g., 90 days).
- Required: Successful completion of mandatory Child Welfare training (e.g., DCFS Foundation Training, CANS, etc.) and ability to obtain child welfare licensure/certification as required by the state.
- Valid driver's license, proof of automobile insurance, and a reliable vehicle for travel to clients' homes and community locations is generally required.
