Job Overview
Visiting Nurse Health System is hiring an Wellness Nurse for our Wesley Woods Towers facility. This role will fall under the direct supervision of the Director, Senior Living Care Services and the Sr. Living Care Coord Coordinator, this position is responsible for implementing the medical components of a total wellness program for geriatric residents of a retirement community. The wellness program includes preventative health maintenance and wellness services.
What You’ll Do:
- Resident Care:
- Operates and staffs the Wellness Clinic during specified office hours.
- Performs wellness checks for residents, which includes blood pressures, temperatures, weight checks and minor first aid.
- Serve residents during “open clinic hours”: for light dressing changes, blood pressure/blood oxygen/heart rate checks, weight checks, (AKA light first aid).
- Provide individual medication management consulting.
- Listens and addresses resident concerns regarding their health and wellbeing.
- Identifies residents who are at risk or require assistance and intervenes as necessary.
- Notifies resident, family, and physicians of any changes in residents’ condition.
- Conducts house calls to residential apartments when necessary.
- Follows up with residents that are ill and may require medical attention, including residents returning from the hospital.
- Responds to emergency calls when on duty.
Resident Services Coordination:
- Coordinate and support transitions from hospital/rehab back to WWSL (in tandem with Service Coordinators as applicable).
- Provides residents with community resources that may be available to help meet their needs and enhance their quality of life.
- Assists residents with arrangements of home health services, rehabilitative service or other services that may be needed by the resident.
- Assists residents with medication management or review, especially after changes to prescriptions or recent illnesses.
- Serves as a liaison between residents, families, and physician offices.
- Assists in the planning and implementation of healthcare related community events.
- Works as a part of the interdependent Wellness Team, including Service Coordinators, Life Enrichment Directors, and other Wellness Staff.
Documentation:
- Develops and maintains healthcare files for residents.
- Updates resident emergency information as obtained from residents.
- Provides Wellness Team with logs of consultation time and services provided.
Education:
- Give group-based general education classes/bulletin boards/newsletter articles regarding health, wellness and disease.

You’re Perfect for the Position if you:
- Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).
- Hold current licensure by the Georgia State Board of Nursing and valid CPR certification.
- Must have and demonstrate excellent clinical skills.
- Must excel in managing client relationships. Strong client-facing skills are essential.
- Must have Sr. Level RN patient transitional experience.
- Strong customer service skills are required when working with patients, their families, and medical facility team members.
- Must thrive in a high-visibility, client-facing environment.
- Possess a valid driver’s license, reliable transportation, and current auto insurance.
- Demonstrate knowledge of Medicare, Medicaid, and funding source regulations as applied to hospice/home health care.
- Exhibit advanced clinical nursing skills and comfort collaborating with interdisciplinary teams.
- Have proven decision-making, time management, and interpersonal skills, functioning independently in a home environment.
- Are proficient with computers and basic technology for documentation, email, HR systems, and online training.
