Affinity is looking for reliable, compassionate, professional Registered Nurses (RNs) to care for our patients in Philadelphia County. The ideal RN professional is highly skilled, organized, and compassionate. We have a variety of opportunities available (e.g., Shift Care, Per Diem, Start of Care (SOC), Skilled Visits, Hourly assignments, etc.) depending on your availability and preferences. We use Axxess and HHAexchange.

Responsibilities
- Record and assess a patient’s medical history accurately
- Monitor and record vital signs including blood pressure, temperature, heart rate etc.
- Perform Start of Care (SOC) visits and ongoing patient evaluations
- Develop and update patient care plans in coordination with physicians
- Monitor patients’ condition and identify progress or side-effects of treatments
- Administer medications, treatments, and IV therapy as prescribed
- Educate patients and families and provide emotional support
- Maintain accurate documentation and ensure compliance with home health standards

Skills
- Proven experience as a Registered Nurse
- Ability to manage patient care independently and within a team
- Familiarity with home health care standards and procedures
- Strong understanding of confidentiality and nursing best practices
- Excellent knowledge of medical terminology
- Good knowledge of electronic charting systems
- Strong clinical assessment and organizational skills
- Excellent communication and interpersonal skills
- Compassionate and able to handle stress
- Active RN license is required
