In addition, many of our chapters provide person-centered and financial planning, recreational activities, and other supports that meet the unique needs of people with I/DD and their families. JOB PURPOSE:The Client Medical Liaison serves as a critical bridge between healthcare providers, clients, and internal teams to ensure the delivery of high-quality medical services and support. This role involves understanding client needs, interpreting complex medical information, and facilitating effective communication to optimize patient outcomes. The liaison will collaborate closely with medical professionals, care coordinators, and administrative staff to streamline processes and resolve any issues that arise. By maintaining up-to-date knowledge of healthcare regulations and industry best practices, the liaison ensures compliance and promotes continuous improvement. Ultimately, this position aims to enhance client satisfaction and foster strong, trust-based relationships within the healthcare environment. - Act as the primary point of contact for clients regarding medical inquiries and service coordination of specialized medical treatment.
- Facilitate communication between healthcare providers, clients, and internal teams to ensure clarity and accuracy of medical information.
- Records patient medical history, vital statistics, and information, such as test results in medical records.
- Prepares treatment rooms for patient examinations, keeping the rooms neat and clean.
- Interviews patients to obtain medical information and measures their vital signs, weight, and height.
- Shows patients to examination rooms and prepares them for the physician.
- Prepares and administers medications as directed by a physician.
- Coordinate and schedule medical appointments, follow-ups, and consultations as needed.
- Assist in the interpretation and explanation of medical reports, treatment plans, and healthcare policies to clients.
- Monitor client cases to ensure timely delivery of services and address any concerns or barriers to care.
- Collaborate with clinical staff to gather and relay relevant patient information while maintaining confidentiality.
- Stay informed about changes in healthcare regulations, insurance policies, and medical advancements to provide accurate guidance.
- Support the development and implementation of client education materials and programs.
- Document all client interactions and maintain detailed records in compliance with healthcare standards.
- Utilizes strong communication skills daily to translate complex medical information into understandable terms for clients and internal teams, ensuring clarity and trust.
- Analytical skills are essential for reviewing medical records, identifying client needs, and coordinating appropriate care plans.
- Organizational skills support managing multiple client cases simultaneously, scheduling appointments, and maintaining accurate documentation.
- Interpersonal skills foster collaborative relationships with healthcare providers, clients, and administrative staff, promoting a seamless care experience.
- Proficiency with healthcare technology and software enables efficient data management and compliance with regulatory standards.
- CMT certification required
- Minimum of 3 years direct support experience or DDA/healthcare experience, preferably in a client-facing or liaison role.
- Strong understanding of medical terminology, healthcare systems, and patient care processes.
- Excellent communication and interpersonal skills to effectively interact with diverse stakeholders.
- Proficiency with electronic health records (EHR) systems and standard office software. Physician medication order sheet and experience with preparation of records and follow up is required.
- Must pass criminal background check and additional nursing CJIS background needed for position.
- Must have years of driving experience with a clear driving record, car insurance and vehicle to drive our individuals.
- The noise level in the work environment is usually moderate
- Usual office working conditions.
