Access TLC Health Care is currently seeking a dedicated and compassionate Medical Social Worker (LCSW) to join our Home Health Department. The Medical Social Worker provides psychosocial assessments, support, resources, and care coordination to patients and their families/caregivers in the home setting.
The Medical Social Worker will assess patients' psychosocial needs and home environment, develop appropriate interventions, provide information and referrals, and collaborate with nurses, physicians, and other members of the healthcare team to support quality patient care.
All charting is completed electronically, and Access TLC will provide you with an internet-enabled iPad.
Per Diem Pay- $75.00 per Evaluation
- $70.00 per Follow-Up Visit
- $35.00 per hour for meetings and orientation
Requirements
- Current California Licensed Clinical Social Worker (LCSW) license through the Board of Behavioral Sciences or a Master's degree in Social Work (MSW)
- Minimum of 1 year of healthcare experience.
- Home health experience preferred.
- Spanish speaking preferred.
- Current CPR certification.
- Valid California driver's license.
- Satisfactory driving record.
- Reliable vehicle with automobile insurance meeting state and/or organizational requirements.
- Strong verbal and written communication skills.
- Excellent organizational and time-management skills.
- Ability to work independently and effectively as part of an interdisciplinary healthcare team.
Requirements:
